The following information is requested:

• Age & Gender
• Ethnicity & Religion
• Legal Status
• Background - including details of current and previous placements
• Behaviour(s) exhibited
• Contact plans
• Education - including school details and history of statementing and exclusions
• Matching issues e.g. type of home required - whether other children can be part of household etc.

We will respond promptly to your call and help whenever we can. If an appropriate vacancy is available, a copy of the Form F and the current review will be provided. A full written background on the child will be requested.
